USER-FRIENDLY GUIDE TO ACCESSING THE 5 FREE USES OF THE ON-LINE BEHAVIORAL EVALUATION CENTER

 

1.      After clicking on the Behavioral Evaluation Center button on the Checkmate Plus homepage, click on “Register” and complete the required information.

 

2.      If you qualify, you will receive an email containing an Administrator number and a temporary Password.

 

3.      As indicated in the email message, go to: https://www.eTestingNetwork.com/Checkmate/admin.asp

 

4.      Fill in the Administrator ID and Password you were given.

 

5.      License Agreement screen: Read and at the bottom click “I agree.”

 

6.      Change your Password.

 

7.      Welcome screen: From side menu click on “Order.”

 

8.      Menu of Symptom Inventories: Decide on a Symptom Inventory to try out (e.g., Child Symptom Inventory-Parent Checklist), and click on “Order” to the left of the Inventory.  Ignore the fee because you will later enter the Promotion Code entitling you to 5 free uses.

 

9.      Order Product: In the Quantity Ordered box, enter “1” and click on “Submit.”

 

10.   Confirm Order: You now need to “order” 4 more free uses at this time, so click on “Add Test” at the bottom of the screen.

 

11.   Product Catalog: Click on the next Symptom Inventory you would like to try.  Repeat steps 8 to 10 until you have ordered a total of 5 Inventories.

 

12.   Confirm Order: Enter the Promotion Code you were given in the space indicated and click “Check Out.”

 

13.   Confirm Order: You should see all the Inventories you ordered and all the charges deducted.  Click “Check Out.”

 

14.   Order Confirmation: Follow the screen directions to click on “Assign Tests.”

 

15.   Ordered Tests: Inventories you ordered should appear under “Test Name.”  Fill in “1” under “Qty to Generate” next to the Inventory you would like to try first, and click on “Submit.” [A Tester ID will be generated for that Inventory.]

 

16.   Tester Assignment and Notification: At the bottom of the screen, fill in a First Name, Last Name, and email address of the adult (or adolescent in the case of the Youth’s Inventory) who will complete the Inventory for a child or adolescent. You should fill in your name and email if you are trying out the system because you will see how the Inventories look on-line.  Also enter a sample child’s name in the “Test Subject” box [can be a made up name.]  Click “Notify” if you want to see what the email notification (instruction how to access the Inventory) looks like (sent to parent or teacher who completes Inventory).

 

17.   Tester Assignment and Notification: You can click on Tester ID and take the Inventory you selected.

 

18.   BEC Test Login comes up with Tester ID and Administrator ID filled in. Click on “Submit.”

 

19.   The Inventory appears. The name of the Test Subject you indicated is filled in for you.  You have to indicate the gender of child being rated. Click “Submit.”

 

20.   Test Instructions: Click “Start the Test.”

 

21.   Items appear: Click on response choices.

 

22.   At the end of the Inventory, click on “Finish Test.”

 

23.   Click on either “Submit” (if you want to submit) or “Review” (returns to page 1 of Inventory so you can look over your responses before submitting).

 

24.   To see the results of the Inventory you just completed: On side menu click on “Current Status.” You should see the name of the Inventory you just completed, and in the Completed column, there should be a “1.”  Click on the “1.”

 

25.   Completed Tests: At the bottom of the screen you should see the name of the Inventory you completed and the Tester ID assigned to it.  Click on the Tester ID number (T####).

 

26.   The results of the Inventory you completed should appear. You can print it by clicking on the printer icon.

 

27.   To complete another Inventory, on the side menu click on “Current Status.”

 

28.   Current Test Status: Under “Assigned” column, click on “1” for the next Inventory you would like to try out from those you ordered.

 

29.   Assigned Tester IDs: Click on the Tester ID number assigned.

 

30.   Tester ID Information: You can either send the test to someone else by entering their contact information (their First Name, Last Name, and email address) and click the “Notify” box OR click on the icon to take the test immediately on your own computer.

 

31.   If you clicked the icon, the Tester and Administrator IDs are already filled in. Click “Submit.”

 

32.   Fill in the information for the child/adolescent you are rating.

 

33.   Follow instructions above for completing and accessing the results (#19-#26).

 

34.   Your account can be accessed to complete or assign the remaining Inventories you ordered for your free trials by going to the Behavioral Evaluation Center on the Checkmate Plus web site. Sign in with your assigned Administrator # and your personalized Password. Click on “Current Status” on the side menu. You can access Inventories you Ordered (ordered but did not assign yet to anyone to complete) and Assigned (ordered and assigned to be completed but did not activate yet) by clicking on the “1” under one of these columns.  You can also access the results of any Inventories that were completed by clicking on the “1” under the Completed column.
