The Adolescent Symptom Inventory-4 (ASI-4) is a behavior rating scale that screens for a variety of adolescent psychiatric disorders and is based on the criteria specified in the Diagnostic and Statistical Manual of Mental Disorders-IV (American Psychiatric Association; 1994). The ASI-4 is appropriate for use with male and female adolescents aged 12-18 years old in middle school or junior or senior high school.

ASI-4 scores show a high degree of correspondence with psychiatric diagnosis (criterion validity) and correlate well with other commonly used dimensional scales of psychopathology (convergent/divergent validity). As a result, the ASI-4 is useful as a screening instrument in mental health settings, schools, and healthcare facilities.

Three versions of the ASI-4 are available. The ASI-4 Parent Checklist contains 120 items that screen for 18 emotional and behavioral disorders. The ASI-4 Teacher Checklist contains 79 items that screen for 15 emotional and behavioral disorders. Each of these instruments is designed to be completed in less than 20 minutes. The Youth's Inventory-4 (YI-4) is a self-report instrument completed by the adolescent that corresponds item-by-item to the ASI-4 Parent Checklist.

What the ASI-4 Measures

The ASI-4 screens for the following disorders:

	AD/HD		Dysthymic Disorder
	Oppositional Defiant Disorder		Bipolar Disorder
	Conduct Disorder		Schizophrenia
	Generalized Anxiety Disorder		Motor Tics
	Social Phobia		Vocal Tics
	Separation Anxiety Disorder		Bulimia
	Obsessive-Compulsive Disorder		Antisocial Personality Disorder
	Specific Phobia		Anorexia Nervosa
	Panic Attack		Schizoid Personality Disorder
	Major Depressive Disorder		Drug Use

Both Symptom Count scores (diagnostic model) and Symptom Severity scores (normative data model) are reported.
